RN CARE MANAGEMENT
The Care Management Nurse assesses, plans, facilitates, coordinates, monitors, and evaluates options and comprehensive services across the continuum of care, providing for safe, clinically appropriate discharge or transfer of all patients admitted to the Hospital. Education, Required Licenses/Certifications: Graduate of an accredited school of nursing with a current license as a registered nurse (RN) or Licensed Practical Nurse (LPN) by the New Hampshire State Board of Nursing. RN license strongly preferred. BSN degree preferred. Notary Public preferred. Work Experience, Skills, Knowledge and Abilities: Minimum 3 years of acute care hospital nursing experience preferred. Prior experience in skilled nursing facility or intermediate care facility desirable. Prior experience in utilization review or care management preferred. Case Management Certification preferred. Ability to develop and maintain effective relationships and partnerships, as well as ability to engender respect and trust, with medical and administrative staff, patients, and the public. Ability to negotiate, influence, and navigate in a politically-complex environment. Excellent organizational communication skills. Flexibility, accountability, and ability to prioritize and re-prioritize work tasks. Ability to work independently and as a member of a multidisciplinary team.Understanding of health insurance, provider payment methodologies, quality improvement/performance improvement principles, regulatory requirements, and care management guidelines preferred. Understanding of and experience with clinical pathways preferred. Committed to serve as a champion for individualized, pro-active care management and appropriate use of swing services. Experience in process improvement, project management, and change management desired. Knowledge of computer programs, applications, and Meditech Health Information System preferred. Familiarity with computer applications required. Must have technical aptitude and a willingness to learn software applications.
