JOB TITLE: Triage Nurse- Registered Nurse

REPORTS TO: Interim President and CEO

Schedule: Monday-Friday, 8:00 AM - 4:30 PM

 

SUMMARY: Position Summary: We are seeking a skilled and compassionate Triage Nurse to join our team in a remote or hybrid capacity. Under the supervision of the Interim President and CEO and in alignment with agency policies, the Triage Nurse will be responsible for managing and triaging all incoming calls from patients, caregivers, staff, and referral sources. This role is essential to ensure timely, safe, and effective care coordination across the agency.

The ideal candidate is organized, calm under pressure, clinically experienced, and committed to clear communication and exceptional service.

 

QUALIFICATIONS:

Qualifications:

· Active RN license in the state of NH (or compact state)

· Minimum of 2 years of nursing experience (home health or hospice experience preferred but not required)

· Strong clinical judgment and problem-solving skills

· Excellent written and verbal communication skills

· Proficiency with electronic health records and remote technology

· Ability to work independently and manage time effectively in a remote environment

Education and/or Experience:

-Is physically and emotionally capable of performing the job requirements -Licensed, insured ($100,000/$300,000 limits of liability) and reliable automobile -Valid Driver’s license

Able to satisfactorily meet all Post-Offer Pre-employment requirements (including physical capacity exam, background checks, etc.)

 

Licenses/Insurance:

Access to operable, insured motor vehicles and valid driver’s license with acceptable record of safe driving practices.

 

Key Responsibilities:

· Triage and respond to incoming calls in a timely, professional, and empathetic manner

· Assess patient needs over the phone and provide appropriate guidance or escalation

· Collaborate with clinical staff, providers, and administrative teams to coordinate care

· Accurately document all communication and follow-up steps in the medical record

· Support after-hours coverage, including weekends or holidays if needed to support the

· Assist with intake and referral coordination when appropriate

· All other duties as assigned

To support your team during times like vacations, occasional weekend and evening coverage may be required with planned notice.
