Job Summary:

The Clinical Liaison will work closely with referral sources and the NCHHHA Intake team to identify prospective patients and provide them with details about NCHHHA’s offerings and programs. The Clinical Liaison will round routinely at the North Country Healthcare affiliate hospitals and participate in meetings to assist with transitions of care to the home setting. Exceptional customer service and communication skills will be necessary for the Clinical Liaison to ensure referral sources and prospective patients needs are exceeded.

  • Current LPN or RN
  • Minimum of 2 years experience in healthcare, home health and hospice experience preferred but not required (willing to train)
  • A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position
  • Access to operable, insured motor vehicle and valid driver’s license with acceptable record of safe driving practice

 

REQUIRED SKILLS AND ABILITIES:

  • Exceptional customer service skills.
  • Excellent verbal and written communication skills.
  • Ability to learn and master Home Health, Palliative and Hospice regulatory requirements for admission to each program
  • Must demonstrate the following personal attributes including, but not limited to, being compassionate, assertive, enthusiastic, honest, ethical and show humility.
  • Proficient with Microsoft Office Suite and related software.

ESSENTIAL FUNCTIONS:

  • Able to perform intake functions which include but are not limited to: reviewing referrals, identifying opportunities for added disciplines to maximize patients’ success, identify regulatory components that are required for referrals to be complete, build charts in the electronic medical record
  • Attend affiliate hospital inpatient discharge and readmission meetings to discuss services and patient needs
  • Round, in person at each affiliate hospital to meet patients/families and discuss their discharge plans and appropriate programs to help make them successful at home
  • Identify barriers and assist with problem solving procedures which will allow for safe and timely discharges to home.
  • Collaborate and communicate regularly with Case Management departments and referral sources.
  • Track referrals and identify areas for improvement, compile data and share with Executive Team and affiliate hospital Presidents
  • Assist with office triage and clinical needs that arise throughout the day
  • Assist with field visits and provide patient care as appropriate
  • Maintains Agency drop site locations and ensures appropriate and adequate supplies are available for staff
  • Assists with community events to ensure community is aware of program offerings, this includes attending events, assessing needs for clinical participation in area clinics, health fairs etc.
  • Maintains a good working relationship with community referral sources.
  • Aids in the development of educational and in-service trainings in collaboration with Director.
  • Serves as the administrator on call when scheduled.
  • Assures compliance with all local, state, and federal laws regarding licensure and certification of organization personnel.
  • Perform other duties as assigned.

LIMITED OR RESTRICTED FUNCTIONS/RESPONSIBILITIES:

The following functions and responsibilities will NOT be provided by the Clinical Manager:

  • Transportation of clients.
  • Acting as a guardian or Power of Attorney for an agency client
  • May not sign checks
  • Contracting in a private duty capacity with an active Agency client.
  • Other functions/responsibilities as may be restricted
