Evening RN On-call with visits
Position Summary
The Evening On-Call Registered Nurse (RN) provides after-hours coverage for patients receiving home health and hospice services. This role is designed to support both scheduled afternoon visits and after-hours visits as needed. The RN ensures timely response, continuity of care, and compassionate support for patients and families outside of regular business hours.
Essential Duties & Responsibilities
On-Call
· Respond promptly to the on-call triage nurse.
· Perform telephone triage when the triage nurse is unavailable, assessing urgency and determining appropriate interventions.
· Provide education, support, and reassurance to patients and families during after-hours situations.
· Communicate with providers and support staff as necessary.
Patient Visits
· Provide in-home visits for urgent and non-urgent patient needs, including pain/symptom management, wound complications, changes in condition, death visits, and time-specific admissions.
Clinical & Administrative
· Ensure all clinical documentation meets regulatory, compliance, and agency standards.
· Communicate with day-shift staff for smooth handoff of patient care issues.
· Maintain professional boundaries, confidentiality, and compliance with HIPAA.
Qualifications
Education
· Graduate of an accredited school of nursing.
Licensure/Certification
· Current and valid RN license in the state of practice.
· Valid driver’s license and reliable transportation.
Experience
· Minimum of 1–2 years of clinical nursing experience (home health, hospice, ER, or acute care preferred but not required).
· Prior triage or on-call experience strongly preferred.
Skills & Competencies

· Proficiency in EMR documentation and technology use.
· Flexibility, reliability, and strong problem-solving ability.
Work Environment & Schedule
· Scheduled visits beginning at 2:00 PM, with on-call coverage from 4:30 PM – 8:00 AM, Monday through Thursday.
· Includes holiday coverage (1 major and 1 minor holiday per year).
· Requires travel to patient homes, facilities, and hospitals within the service area in all weather types
