POSITION SUMMARY:
The Flex Registered Nurse (RN) is responsible for providing skilled nursing care across both the home health and hospice service lines, tailored to patient needs and aligned with agency priorities. This versatile position demands clinical proficiency, adaptability, and effective communication skills to ensure the delivery of high-quality, patient-centered care in various home environments. The Flex RN supports a range of visit types without case management responsibilities.
ESSENTIAL QUALIFICATIONS
Education:
- Graduate of an accredited nursing program.
Certification:
- N/A
??Licensure:?
- Active RN license in the state of practice is required.
- Association* accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date.

??Work Experience:?
- At least 1 year of nursing experience is required; experience in home health and/or hospice is strongly preferred.
ESSENTIAL FUNCTIONS:
- Perform comprehensive nursing assessments and implement skilled interventions across home health and hospice settings.
- Deliver care according to physician orders, the plan of care, agency protocols, and patient goals.
- Demonstrate proficiency in symptom management, wound care, disease management, end-of-life care, and patient education.
- Maintain awareness of home health eligibility and hospice terminal diagnosis/prognosis requirements.
- Flexibly cover home health and/or hospice assignments based on staffing needs, patient acuity, and visit volume.
- Accept patient visits in varied geographic areas within the agency’s service region.
- Participate in weekend, holiday, and on-call rotations as required.
- Complete timely and accurate clinical documentation in the electronic health record per CMS regulations.
- Collaborate effectively with interdisciplinary team members, including physicians, aides, social workers, chaplains, and therapists.
- Communicate effectively with patients, caregivers, referral sources, and internal teams regarding changes in condition and care needs.
- Engage in team meetings, care conferences, and staff training as appropriate.
- Travel frequently within the service area; maintain a valid driver’s license and reliable transportation.
- Adapt to varying home environments and manage potential exposure to infectious conditions.
- Uphold compassion, integrity, and accountability in all interactions, maintaining professional boundaries and patient confidentiality.
- Perform other duties as assigned to contribute to the overall success of the agency.
