POSITION SUMMARY:
The Hospice Clinical Manager is responsible for overseeing the day-to-day clinical operations of the hospice program to ensure high-quality, patient-centered care that is compliant with federal, state, and agency standards. This role provides direct supervision to hospice support staff, facilitates interdisciplinary collaboration, and supports clinical excellence through education, mentorship, and accountability.
ESSENTIAL QUALIFICATIONS
Education:
· Pass the National Council of State Boards of Nursing (NCSBN) Exam and NCLEX-PN
· Complete 1 year of LPN Program. Nursing Degree Preferred.
Licensure:
· LPN License, RN License Preferred: Possess a valid Licensed Practical Nurse (LPN) or Registered Nurse (RN) license issued by the State of New Hampshire
· Association* accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date.
Skills:
· Clinical Expertise: Comprehensive understanding of hospice care standards and practices.
· Communication: Excellent verbal and written communication skills to interact effectively with staff, patients, and families, ensuring a positive experience with the agency.

· Technical Proficiency: Competence in using EMR systems and other relevant software.
Work Experience:
· Minimum of 3-5 years of experience in hospice, or a related field.
Insurance Factors:
· Vehicle Insurance: Access to an operable, insured motor vehicle with a valid driver’s license and acceptable record of safe driving practices.
· Personal Liability Insurance: Licensed and insured with $100,000/$300,000 limits of liability.
ESSENTIAL FUNCTIONS:
1. Care Coordination: Oversee and coordinate patient plan of care to ensure comprehensive and individualized care.
2. Regulatory Compliance: Ensure all hospice clinical practices comply with state, federal, and local regulations, as well as agency policies.
3. Quality Assurance: Monitor and evaluate the quality of hospice patient care through audits and quality assurance programs. Initiating QAPI initiatives related to data trends.
4. Billing Compliance and Pre- billing coordination.
5. Provider Support with ordering and Face to Face coordination.
6. Admission, Recertification and Discharge audits ensuring compliance.
7. Submissions of the HIS, 485s and other necessary documentation.
8. Enclara Pharmacy and Medline oversight ensuring fiduciary responsibilities.
9. Patient Assessments: Oversee initial and ongoing patient assessments to ensure timely and appropriate care delivery, including high acuity patient cases and nursing home hospice coordination.
10. Triage and clinical support.
11. Interdisciplinary Team Meetings: Ensure accuracy of hospice census along with the daily auditing for regulatory compliance
12. Documentation Standards: Ensure all patient care documentation is accurate, complete, and timely.
13. Resource Allocation: Manage clinical resources effectively to meet the needs of patients and the team.
14. Patient and Family Education: Provide education and support to patients and families regarding their care and treatment.
15. Oversee the hospice admission process, including pre-admission screening and certification of terminal illness (CTI) workflow.
16. Coordinates with physicians, facilities, referral sources, and vendors for seamless care delivery.
17. Supports survey readiness through internal audits and quality improvement initiatives.
18. Participate in Administrative On- Call duties
