JOB SUMMARY

The North Country Home Health and Hospice Intake RN will work closely with the Intake and Scheduling team to provide intake and triage coverage for the Agency. The Intake RN will receive and provide clinical review for all incoming referrals for Home Health, Palliative and Hospice. The primary focus of review will be to ensure referrals are processed in a timely manner, have a clinical/skilled need and are in line with all regulatory components. The Intake RN will also serve as a liaison between NCHHHA and referral sources.

Essential Qualifications: Current NH RN license with 2-years recent experience as an RN in community health, medical/surgical in an acute or skilled long term care setting, ability to read and interpret written documents, computer competency including the ability to retrieve information necessary to complete job responsibilities,  Excellent multi-tasking abilities with the ability to simultaneously manage multiple tasks at one time, and customer service skills. Must be able to work in a team and/or independently. Excellent oral/written communication and interpersonal skills. Proficient Microsoft Office Skills and data entry. Physically and emotionally capable of performing the job requirements.  Licensed, insured ($100,00/$300,000 limits of liability) and a reliable automobile, and a valid driver's license is required.
