Licensed Practical Nurse (Full-Time)
Job Summary:
As an LPN at NCHHHA, you will have the ability to get to know your individual patients and support your team members in executing tailored plans of care for Home Health and Hospice patients. You will perform comprehensive assessments and provide skilled nursing care for each patient under the supervision of and in partnership with a qualified Registered Nurse. Our LPNs are an integral part of a dynamic, multi-disciplinary team. You will maintain a high level of autonomy and work to the top of your license. You will enjoy a flexible workday, with an option to start and end your day from home. Strong clinical support and educational resources provided as well as support in obtaining certifications in your area of expertise.
Essential Qualifications:
- Must have current active NH LPN License
- Minimum of 1 year experience in healthcare setting.
- Ability to demonstrate flexibility in response to unexpected changes in work volume and work schedule, required
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability or financial condition of the patient.
- Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff and interact professionally as a member of a multi-disciplinary team.
- Speak, write, read and comprehend English.
- Computer proficiency in electronic medical records data entry.
- Maintains valid driver’s license and certificate of auto insurance coverage.
- Current BLS Certification required.

Please Note: All new hires are required to attend a mandatory three-day orientation at Weeks Medical Center in Lancaster, NH. This orientation provides essential training, compliance information, and an introduction to our organizational culture to ensure a smooth and successful transition into your role. Attendance on all three days is required for onboarding completion.
Additionally, if Weeks Medical Center is not your designated home base, you will be eligible for travel reimbursement in accordance with our standard travel policy.
