POSITION SUMMARY:  

The System Director of Home-Based Care is responsible for overseeing and leading the strategic and operational functions of North Country Home Health and Hospice Agency (NCHHHA) and home-based care throughout North Country Healthcare (NCH). This role ensures the delivery of high-quality, patient-centered care to individuals in the comfort of their homes while managing a team of clinical and operational professionals. The director will collaborate with leadership across the healthcare system to align home-based care services with organizational goals, regulatory requirements, and best practice standards. The director will serve as a non-voting member on the NCHHHA Board of Directors to ensure the mission, vision, and values of the Agency are continually being met.  

 

ESSENTIAL QUALIFICATIONS 

Education: 

  • Associates or Bachelor's degree in Nursing, Healthcare Administration, or related field (required). 
  • Master’s degree in Healthcare Administration, Business, or a related field (preferred). 

 

Certification: 

  • Registered Nurse (RN)  
  • Certification in Home Care or Healthcare Management. 

 

??Licensure:? 

  • Association* accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date. 

Skills: 

  • Exceptional leadership and interpersonal skills, with the ability to motivate and inspire diverse teams. 
  • Strong understanding of healthcare delivery models, particularly home-based care and its integration with other healthcare services. 
  • Excellent communication, organizational, and problem-solving skills. 
  • Demonstrated ability to manage budgets and financial performance in a healthcare setting. 
  • Ability to foster collaborative relationships with internal and external stakeholders. 

 

??Work Experience:? 

  • Minimum of 7-10 years of experience in healthcare leadership, with at least 5 years in home-based care or a similar field. 
  • Proven experience in strategic planning, operational management, and team leadership. 
  • Strong knowledge of healthcare regulations, compliance standards, and payer requirements  

 

ESSENTIAL FUNCTIONS:  

 Strategic Leadership: 

Lead the development, implementation, and evaluation of home-based care services across the system. 

Collaborate with senior leadership to create and execute strategic plans to enhance home-based care offerings. 

Drive innovation and continuous improvement to ensure high-quality patient care and operational efficiency. 

Develop and maintain key partnerships with hospitals, physician groups, payers, and community organizations to expand the reach and effectiveness of home-based care services. 

 

Operational Management: 

Oversee day-to-day operations of home-based care programs, ensuring adherence to budgets, staffing requirements, and operational goals. 

Ensure compliance with federal, state, and local regulations, including Medicare and Medicaid requirements. 

Develop and monitor performance metrics for clinical outcomes, patient satisfaction, cost-effectiveness, and service delivery. 

Manage the integration of home-based care with other healthcare services within the organization. 

 

Clinical Oversight: 

Ensure the provision of high-quality clinical care in the home setting, working closely with clinical teams: nursing, therapy, social work, providers etc. 

Collaborate with the Hospice Medical Director to ensure best practice, clinical quality, and patient satisfaction are being met 

Monitor and assess clinical outcomes, utilizing evidence-based practices and quality improvement initiatives. 

Provide guidance on patient care, ensuring care plans are individualized and patient-centric. 

 

Team Leadership: 

Lead, mentor, and develop a multidisciplinary team of healthcare professionals, including nurses, providers, social workers, therapists, and support staff. 

Promote a culture of collaboration, accountability, and continuous learning among staff members. 

Recruit, train, and retain top talent, ensuring staffing levels are optimized to meet patient needs. 

 

Financial and Resource Management: 

Manage budgets, resource allocation, and financial performance of home-based care services. 

Identify opportunities for cost containment and revenue enhancement, optimizing the use of resources to deliver effective care. 

Collaborate with finance teams to ensure appropriate reimbursement for services provided. 

 

Patient and Family Engagement: 

Champion a patient- and family-centered approach to care, ensuring that patients and their families receive personalized, compassionate services. 

Address concerns, feedback, and complaints in a timely and professional manner. 

Educate patients and families on available home-based care options, treatment plans, and self-care strategies. 

 

Quality Improvement and Compliance: 

Lead continuous quality improvement initiatives to enhance patient outcomes, satisfaction, and operational efficiency. 

Ensure the home-based care program meets or exceeds all regulatory standards, including accreditation and certification requirements. 

Stay current with industry trends, best practices, and emerging technologies related to home-based care.
