Job Summary:

The Care Management Nurse assesses, plans, facilitates, coordinates, monitors, and evaluates options and comprehensive services across the continuum of care, providing for safe, clinically appropriate discharge or transfer of all patients admitted to the Hospital. 

Essential Qualifications:  Graduate of an accredited school of nursing with a current license as a registered nurse (RN) or Licensed Practical Nurse (LPN) by the New Hampshire State Board of Nursing. RN, BSN license is strongly preferred. Notary Public and Case Management certification is preferred. Minimum 3 years of acute care hospital nursing experience preferred, prior experience in a skilled nursing facility or intermediate care facility is desirable. Prior experience in utilization review or care management is preferred.  Must have the ability to develop and maintain effective relationships and partnerships, as well as the ability to engender respect and trust, with medical and administrative staff, patients, and the public, and to negotiate, influence, and navigate in a politically-complex environment. Excellent organizational communication skills. Flexibility, accountability, and ability to prioritize and re-prioritize work tasks. Ability to work independently and as a member of a multidisciplinary team. Understanding health insurance, provider payment methodologies, quality improvement/performance improvement principles, regulatory requirements, care management guidelines, and experience with clinical pathways is preferred. Committed to serving as a champion for individualized, proactive care management and appropriate use of swing services. Experience in process improvement, project management, and change management desired. Knowledge of computer programs, applications, and Meditech Health Information System preferred. Familiarity with computer applications is required. Must have technical aptitude and a willingness to learn software applications.
