Job Description
The RN Patient Services Coordinator is an integral part of the healthcare team, responsible for overseeing patient care services and ensuring the highest quality of patient-centered care. ESSENTIAL QUALIFICATIONS
Education:
- Associates of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) or Master's degree in Nursing or Healthcare Administration preferred.
Certification:
- Certification in Case Management (CCM) or similar is an advantage.
Licensure:
- Current Registered Nurse (RN) license in the appropriate state is mandatory.
- Association* accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date.
Skills:
- Strong clinical evaluation and patient assessment skills.
- Excellent organizational and time-management abilities.
- Proficient in healthcare technology and electronic health records (EHR).
- Outstanding communication and interpersonal skills.
- Critical thinking with problem-solving capabilities.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficiency in patient education and advocacy.
- Leadership and staff management skills.
Work Experience:
- Minimum of three years of clinical nursing experience required.
- Previous experience in case management, patient coordination, or a similar healthcare role preferred.
- Experience in a supervisory or leadership position is a plus.
ESSENTIAL FUNCTIONS:
- Coordinate and oversee the delivery of patient care services to ensure high standards of practice and quality care.
- Serve as the primary liaison between patients, families, and the healthcare team to facilitate communication and continuity of care.
- Conduct comprehensive assessments of patients’ needs and develop individualized care plans.
- Monitor and evaluate patient outcomes, reporting any significant changes to the respective healthcare professionals.
- Collaborate with physicians and other healthcare providers to optimize treatment plans.
- Manage and resolve patient and family concerns in a timely and effective manner.
- Provide patient education and resources to promote informed decision-making and self-management.
- Ensure compliance with healthcare regulations and institutional policies.
- Participate in quality improvement initiatives to enhance patient services and outcomes.
- Supervise and support nursing staff, providing guidance and performance feedback as necessary.
- Facilitate the seamless transition of care, coordinating discharge planning and follow-up services.
- Maintain accurate and comprehensive patient records using EHR systems.
- Participate in multidisciplinary team meetings and contribute to care planning and case discussions.
- Develop and implement strategies to improve patient satisfaction and service efficiency.
- Stay current on industry trends, continuing education, and emerging best practices to incorporate into the patient coordination process.
