POSITION SUMMARY:

The Weekend Baylor Home Health Registered Nurse is accountable for delivering comprehensive nursing assessments, planning, and care with the goal of optimizing the comfort and health of patients and their families in their home environments. This role involves teaching that respects the patient's needs and value system, guided by the interdisciplinary plan of care. The RN assumes full responsibility for the practice of professional nursing in compliance with the NH State Nurse Practice Act. This full-time exempt position requires availability starting Friday at 4:30 pm through Monday at 8:00 am and includes both on-call responsibilities and home visits. This position is based out to the Berlin and surrounding area. 

 

ESSENTIAL QUALIFICATIONS

Education:

· Graduation from an accredited school of professional nursing.

 

Licensure:

· Current Registered Nurse (RN) license in the state of New Hampshire.

· Association* accredited course in Basic Life Support (BLS) (for clinical staff), or Heartsaver CPR AED (for nonclinical staff), and renewal on a regular basis, with up to a three-month grace period after the expiration date.

 

Skills:

· Proficiency in oral and written communication.

· Strong interpersonal skills.

· Excellent organizational and time management capabilities.

· Ability to work independently in a home care setting.

 

Work Experience:

· Previous clinical experience in a healthcare setting is preferred but not mandatory.

 

ESSENTIAL FUNCTIONS:

1. Conduct comprehensive assessments of patients in their home settings.

2. Develop, implement, and evaluate individualized nursing care plans.

3. Provide education to patients and families tailored to their needs and environments.

4. Administer medications and treatments as prescribed and in accordance with care plans.

5. Collaborate with interdisciplinary teams to ensure coordinated and comprehensive care.

6. Document patient assessments, interventions, and responses in a timely and accurate manner.

7. Manage and prioritize multiple patient visits and on-call responsibilities effectively.

8. Offer emotional support to patients and families, respecting their values and preferences.

9. Adjust care approaches and plans based on patients' evolving conditions and feedback.

10. Ensure all care provided meets NH State Nurse Practice Act standards and agency policies.

11. Maintain open and clear communication with patients, families, and healthcare team members.

12. Respond promptly to patient or family emergencies and adapt care accordingly.

13. Conduct patient and family teaching sessions, promoting health maintenance and disease prevention.

14. Participate in ongoing professional development and competency evaluations.

15. Utilize organizational resources efficiently to support patient care and agency operations.
